gear selecter keeps falling off - GS125

on my mates suzuki GS125 (f reg)

the lever slides onto the shaft, then there is still a fair bit of the shaft showing, with a grove cut all the way around it, although it looks a bit big for a ser-clip,

anyone had one/or know whtas needed

How much sticks out ?. And how wide is the grove.
