People do make mistakes, and actually on the scale of things and considering some I've seen, that was a fairly easy one. The arrows on the road are a bit of a give away, but it could genuinely do with a double white line system and no entry signs there to reinforce the message. As for the oncoming lorry on the slip road she would have come across it very quickly and her first thought may well have been "what's that lorry doing coming the wrong way...." She did well to miss it, most drivers just panic and stand on the brakes.... Just lucky it was a lorry she clipped and not a motorcyclist!

I'm not trying to make light of this, I'm just trying to keep things in proportion. The vast majority of drivers (and riders) on our roads pass their test and make no further effort to improve either their physical skills, or their understanding of what happens on the roads. Most of the time they get away with it, every now and then they don't.
