I had some relatively square tyres, been meaning to change them but other things kept breaking and i fixed them instead. Went into a left hand corner and leant off the bike but wasn't really going fast enough but leant over a bit more and i hit that bit of tyre where the square hits the curve and equals zero grip. It slipped out immediately, no warning, no skid mark on the road, so little tyre was in contact it just went.

Bad luck but at least it's something we can all learn from. You're persumably a pretty experienced rider (judging by bike/join date) so I didn't think it would be one of the usual mistakes.

My back tyre's got a bit squared recently and I've noticed it locking under heavy braking where the centre of the tread is worn. Didn't really think it could affect grip when the bike's leant over till I read that and thought about it - will bear that in mind.
