From what I could see he practically fell off the inside of the bike due to the initial slide, it happens. Most likely his foot slipped off the inside footpeg. At that point he had no control over the bike and it went were the laws of physics wanted to take it. At those speeds and proximity, Edwards and Rossi could do nothing about it, I doubt Rossi even knew it was happening until the impact as he would have been unsighted and focussing on passing Edwards.

A very unfortunate freak accident.
