I'll call tomorrow if it will help. I though nuts and bolts coming lose was part and parcel of owning a bike and one of those things you check occassionally. Mine came lose and had started to move off the spline - some difficult gear changes made me check it all over and spot the problem. Having nipped up the little screw holding it on, it is now fine. Perhaps it's one of those checks that should be done at the initial service?
