If it's only rusty on the sides, give it a scrub with parafin, allow to dry, then lube it up again & take it for a run. That should sort surface rust out.

If you're having to adjust the chain every few days (or every week etc), then it's time to change. Any sideways play, or excessive stretch in the chain, and it needs replacing.

I'd always change sprockets at the same time as the chain. In fact, I just did, to the tune of £135.90
