Default New pads won't fit in caliper.

Rear Brake:
Put my new pads in. pistons are pushed in as far as they will go. I have double checked they are the correct pads. The pads wont fit around the disk, and the innermost (closest to the center of the wheel) seems loose.

What could cause these problems ?

Hmm,

I would definitely double check those pistons are fully in.
Are they not moving slightly out as you take pressure off the pistons?

Never heard of this problem before.

there are 'keys' in the pads that sit in the carrier of the calliper.

Are the pistons fully in?If not the new thicker pads may be too big.Try taking the lid off the fluid reservoir to take pressure off the system if you cant get the pads pistons in but be careful of spillage. Its possible you have topped up the fluid to compensate for pad wear over time and there may now be too much fluid.
