The difference is simply that you lose fluid faster than normal for the same effort. Have you been training with energy drinks or just water?
I think I might go for a run/walk strategy if it feels stupidly hot, I don't think that it would slow me down much. I've been trying to take advantage of the warmer days recently to get used to running in the heat.

I only ever drink lucozade (or zym/nuun and sweets) when I'm running longer distances.

You run like a chicken?

Good on you for givinh it a go, but if the knees are bad then you might need to look elsewhere for exercise. Cycling?

Good luck to those in the Marathon on Sunday

I'm rempted to try running as well as cycling but haven't run for years. Think it might do me good but I'll probably hate it too

Do it! You'll hate it to start with and then you'll get hooked

Dont panic, you'll get there. The heat will certainly affect your performance but its important to maintain what you've trained to do, dont change your plan. Go at a pace you are comfortable with, hydrate in advance (start now) and take carbs on board throughout the distance.

The real hard work started at 20 miles for me, it was plain sailing up until then. All down to hydration and fuel.

Cant wait to watch on TV!
