I think mileage would be worth putting on your data entries but:

1. How long you have been riding for (not including months or yrs out)
3-4 years
2. How many hours you ride in a week/month (rough estimates only)
bout 5 a week average over the whole year
3. How many offs you've had on the road(regardless of whose at fault, or if it was just you that binned it) - dont incl track day offs
2 that've been serious enough to report
4. Years after you started riding for each off
1.5, 2
5. The speed of each off
15mph 40mph
6. The location of each off (in terms of urban vs countryside)
urban, erm residentially countrysidey, there were fileds and sheep and vicars about but there were adjacent streets with houses.
7. Time of day of each off (again roughly, no need for exact times)
5.30pm, 5pm
8. Nature of injuries sustained after each off - if no injuries say no injuries.
whiplash, muscle damage sprained wrist, damaged testicles!

screwed my manhood again (worse than the last), fractured pubic ramus(?...stole that off ed, basically pelvis), slight haemothorax

3. How many offs you've had on the road(regardless of whose at fault, or if it was just you that binned it) - dont incl track day offs: Once in 1988
4. How many years of riding you had under your belt at the time of each off: ~1year on the road
5. The speed of each off: ~ 50kph/30mph
6. The location of each off (in terms of urban vs countryside): Suburban (residential)
7. Time of day of each off (again roughly, no need for exact times): 3pm
8. Nature of injuries sustained after each off - if no injuries say no injuries: Broken collar bone, concussion, light road-rash (shorts and t-shirt)

On and off for 18 years

Used to be a lot when I had no cage but these days it's between 1 and 10 hours a week depending on blasts

3. How many offs you've had on the road(regardless of whose at fault, or if it was just you that binned it) - dont incl track day offs

1 off, 2 bumps

4. How many years of riding you had under your belt at the time of each off

1 year ish for the off
2 years for the first bump
13 years for the second

5. The speed of each off

1 - 20 mph
2 - 20 mph
3 - Stopped

6. The location of each off (in terms of urban vs countryside)

1 - backroad between Newton and Kingsteignton, hit a lump of clay just on the other side of a hump back bridge (countryside)
2 - clipped a car coming the other way on a single track hump back bridge going to Bovey Tracey (countryside)
3 - Stopped at a big roundabout coming off the M62, some cage just drove into the back of me (Neither - motorway junction so it was in the countryside but not in the real sense of "The Countryside")

7. Time of day of each off (again roughly, no need for exact times)

1 - 8.30 ish going to work
2 - can't remember (daylight)
3 - Same, daylight - out for a burn

8. Nature of injuries sustained after each off - if no injuries say no injuries.

1,2,3 - nothing really maybe the odd bump but that's it

1. two years on front, fifteen years on Pillion.
2. three hours commuting, two or more hours playing.
3. One only. While under instruction on DAS.
4. Less than a week.
5. Less than fifteen MPH.
6. Roundabout in centre of small town.
7. early afternoon.
8. bruises only, shock and lots of tears.

no injuries (the car broke my fall!)

offs are listed in order. all were accompanied by a sharp, stabbing pain in the wallet.
