My Fiesta (shock, horror) has rusty rear wheel arches.

Oh god the dreaded fiesta wheel rot
My other halfs fiesta went the same way a little while ago. Wire wool, kurust, filler, wet and dry paper, some halfrauds paint and an afternoon made it presentable again. Pretty much all fiestas of a certain vintage go that way though... |

If you want it done properly then new arch is the way to go as Bibio said. If you want it done cheaply then it can be patched up at home. I wouldn't worry about it too much unless it gets into the sill though. But as always, prevention is better than cure

The sills seem solid to me. Am I right in saying the arch itself where it has gone through is not structural and not cause for an MOT fail?

Don't quote me, but from memory I think it will only fail if there is any sharp / jagged edges. the arch itself is not structural.
