I have been using one of these for a year now and it cured my RSI/carpal tunnel problems very quickly. Just as easy to use as the thumb wheeled version once used to it with all the same advantages. There has been a few different versions of it - mine is like the one linked above with forward and back buttons - and keeping your hand centrally over the mouse is (I find) very comfortable for extended use. I've playes several games with it with no problems.
