I only looked a the first pic but it still looks like maintenance/installation issues.

It does look ver dry/rusty (could be wrong) and maybe over Torqued. It should be greased and for some reason(and I could be very wrong here) I think there is an O'ring in there somewhere.

My thought is there is maybe some other underlinging reasons( all all already given by other posters) why it is in such a state.

The only ones I've broken is when I've being a bit hurried with installation and also stepping on one .
