Default Brake pad cracked.

SV650 SK9
So i was replacing the rear wheel after having a puncture repaired. When bringing the rear caliper down on the brake disk, with apparently no real force, the left side pad friction material hits the edge of the brake disk and completely comes away from it's backing material.
After several expletives am resigned to replacing the pads.
Removing the bolt and pin and extracting the pads and inspect the other pad and i see this:





Presume these are OEM pads less than 3 years old.
Looks like this was about to fail too. Delaminated all the way through in the middle.
Fitting replacement pads and there was no other obvious fault e.g. pots moved in and out Ok etc.

So why would this happen? Any ideaas?

Looks fairly corroded, i had this happen on a cars brake disc due to it being corroded.

Mine did the same! Same make and numbers stamped on them. I think the loose bit was loose for a while as the brake was doing weird things.

Disks Not discoloured, flat to the touch, .3mm over minimum thickness stamed on disk, the only thing is there does seem to be a brake pad shaped edge pitting line on the disk as if it had stood for a while.

Didnt have any complaint about the braking before the puncture, Reasonable feel and could lock up the rear.
After running the new pads for a few miles the braking seems similar.
