how tight should the adjuster nut be? just beyond pinched? obviously i don't want to over tighten

you certainly don't, i find if i've pinched the nuts i've probably gone too far, slacken off your grip if you ever want to father children...

Just tight enough to take up play in the bearings, you may find that new bearings will settle slightly too, make sure you use loads of grease, they only rotate slightly so you can pack them fully. (unlike wheel bearings which should be half packed)

Preferably castrol BNS, or a "race" grease IMO, but that's splitting hairs really, normal grease will do the job just not for quite as long.

old races are out, top one shattered whilst i was tapping it out but i cant see any obvious indentations on either top or bottom so must be on the bearings

new races are in the freezer for 30 mins so hopefully they will be easy to knock in
