Name, blood group, national insurance number, allergies, and date of birth.
Everything else, like next of kin, can be got through your GP records. |

I'd not bother with that- my NOK (who are on my tags) know my religious beliefs and type of funeral I'd want.
As you say, even blood type is now redundant unless you're travelling abroad (and if you do, your noc should have the international code in it) as they test before transfusions nowadays. |

True, they can test that in a few minutes plus everybody can except one blood group (can't remember which) which is the first pint they will pump into you before they confirm your actual group. I have a bit of experience in this....
